Question

We prayed in congregation in a mosque, and the imām stood up for a fifth rakʿah. The maʾmūms [those praying behind the imām] said to him, “Subḥān Allāh [Glory be to Allah]”, so he sat down and then gave the taslīm [the closing salām of the prayer] without performing the prostration of forgetfulness, and the worshippers left like that?

Answer

This is a mistake. When a person stands up for a fifth rakʿah in Ẓuhr, ʿAṣr or ʿIshāʾ, is then alerted that it is extra, and goes back, he must perform the prostration of forgetfulness before giving the taslīm, because whenever the Prophet, ʿalayhi al-ṣalātu wa-l-salām, added something to the prayer or left something out, he made two prostrations.

The scholars have stated that if someone does an act that would invalidate the prayer, the prostration of forgetfulness becomes obligatory on him. There is no doubt that standing up for a fifth rakʿah deliberately invalidates the prayer. So if he does it by oversight, he must go back, sit, and recite the taḥiyyāt [the testimony recited while sitting in prayer], then perform the prostration of forgetfulness before giving the taslīm. If he delays it and prostrates after the salām, there is no harm in that.

As for leaving out the prostration, no. Perhaps he then left it out by oversight and was not alerted; but what is obligatory on him in a case like this is to perform the prostration of forgetfulness. The same applies in Maghrib if he stands up for a fourth rakʿah, or in Fajr if he stands up for a third, and is then alerted, or remembers, and goes back: he too must perform the prostration of forgetfulness, to repair his prayer.

Question

What if he sat in the third rakʿah in place of the fourth, and was then alerted?

Answer

He performs the prostration of forgetfulness, because when he sat in the third he had resolved to leave out a rakʿah. So once he is alerted, he prostrates. The Prophet ﷺ said to the man who was unsure whether he had prayed three or four: let him count it as three, then complete his prayer, then perform the prostration of forgetfulness; if he had prayed five, they make his prayer even for him, and if he had prayed four, it is a humiliation for the Shayṭān.

Question

And what if he is alerted after he has left the mosque?

Answer

He goes back and prostrates with those present.
